Plan networks change every year. Give us the full plan name and the member ID from your card and we will check it against the products we are contracted for.
IMS is a PPO network reached through employer plans and third party administrators. We are in it, and we see members age 16 and older at our Southlake office.
Finding your real network name
Most people cannot answer the question “what network are you on,” and that is not a failure on their part. You never bought the network. Your employer did, and then handed you a card branded by somebody else.
Here is where to look, in order.
The back of the card, small print. Look for a phrase like network access provided by, or a second logo near the claims mailing address. This is where it hides most often.
The front, under the main logo. Some cards print it here in smaller type than the administrator’s name.
An old explanation of benefits. If you have had any claim processed, the EOB usually names the network that priced it. This is the most reliable source and the one people never think of.
Your benefits summary. The document from open enrollment names it explicitly, usually in the first two pages.
If none of that works, call us with the member ID. The eligibility response tells us what we need.
Why we would rather check than assume
Two networks with similar names can have completely different contracted provider lists. A practice that says yes based on a name that looks familiar is guessing with your money.
We run the check on the phone and tell you what it returns, including when the answer is no.
What we handle
Annual physicals, preventive screening, chronic conditions, sick visits, women’s health, men’s health, weight management and telehealth.
Patients age 16 and older. No pediatric, prenatal or emergency care.
Costs
Preventive care is covered in full under most plans, and problem-focused visits apply to your copay or deductible. Both can be billed from one visit, which we explain in the room rather than by statement. See why your annual physical came with a bill.

IMS questions we get asked
Where do I actually find the network name on my card?
Usually the back, in small print near the claims address, often next to a phrase like network access provided by. Some cards put it on the front under the logo. If neither, your explanation of benefits from a past claim will name it.
I have never heard of IMS. Is this legitimate coverage?
Yes. Networks rented by employer plans are rarely recognized by the members using them, because the member never buys the network directly. Unfamiliarity is the normal experience, not a warning sign.
What if the card names two networks?
That happens with tiered or wrapped arrangements, where one network is primary and another covers gaps. Read us both and we will tell you which one we are contracted under.
Do I need a referral?
Generally not for primary care. If your plan adds a requirement, your plan documents or the member services number on your card will say so.
