Employer Network Plans We Accept

If your insurance card shows a network name instead of a familiar carrier, your employer is very likely self-funded. These are the networks we are contracted with, and this is how to check whether yours reaches us.

We see patients age 16 and older.

How these plans work

A self-funded employer pays claims directly, uses a third party administrator to process them, and rents a network like one of these to give employees access to contracted providers at negotiated rates. For you the differences are small but worth knowing.

The name on the front of your card may not be your network. Look at the back and in the small print. There is often a separate logo, and that is the one that decides whether we are in network for you.

Your employer sets the benefits, not the network. Your summary plan description is the authority, not any general statement about how the network works.

Verification matters more, not less. We check eligibility before your first visit and tell you exactly what comes back.

Networks we are contracted with

First Health

A national PPO network used by a wide range of employer plans, third party administrators and supplemental products. Many members reach us through First Health without the name ever appearing prominently on the front of their card.

Galaxy Health Network

A Texas based network used by regional employer plans and administrators. If the Galaxy name appears on your card or in your plan documents for provider access, you are in network with us.

Healthcare Highways

Used by a number of North Texas employers, often as part of a plan built around a narrower set of contracted providers in exchange for a lower premium. Narrow networks are exactly why checking beats assuming.

HealthSmart

Both a third party administrator and a network, which is why the name can show up on a card in two different roles. Our contract covers the PPO, HPO and Accel networks.

Imagine Health

Builds curated networks for employer sponsored plans, typically selecting a smaller set of providers rather than contracting broadly. Many practices in this area are not included, so being in network is worth confirming.

IMS

A PPO network reached through employer plans and third party administrators. Members frequently do not recognize the name because it appears in plan documents rather than as the plan brand.

Partners Direct Health

Supports employers contracting directly with providers rather than buying a conventional insured product. Plans built this way often have unusually good primary care benefits, sometimes with reduced cost sharing, because the employer wants people using primary care.

PNOA

A provider network accessed through employer sponsored plans and administrators. Like most network access arrangements, the name may appear only on the back of the card.

What we handle

Annual physicals and preventive screening, chronic conditions including blood pressure, diabetes, cholesterol and thyroid, sick visits, weight management, women's health, men's health, and telehealth where it fits.

We see patients age 16 and older. We do not provide pediatric, prenatal or emergency care.

Costs

Preventive care is covered in full under most plans. Problem-focused visits apply to your copay or deductible, and when both happen in one appointment both are billed. We explain that in the room rather than letting a statement do it. See why your annual physical came with a bill.

Questions we get about network plans

My card does not show any of these names. Am I in the wrong place?

Possibly not. Network logos are often printed small on the back of the card, or named only in your plan documents. Call us and we will help you work out which network your plan uses.

Do I need a referral with a network access plan?

Generally not for primary care. Your plan document is the authority, and we will tell you what the verification shows before you book.

Why is my coverage different from a coworker at another company on the same network?

Because the network only sets who you can see at contracted rates. Your employer sets the benefits. Two people carrying similar looking cards from different employers can genuinely have different coverage.

How do I confirm before my appointment?

Call the office with the plan name, any network names printed on the card, and your member ID. We run the check while you are on the phone and tell you what comes back, including if the answer is no.
