HealthSmart is one of the few names on your insurance card that can mean two entirely different things, which is why this page exists.
We are contracted for the PPO, HPO and Accel networks, and we see HealthSmart members age 16 and older at our Southlake office.
Two roles, one name
As a third party administrator, HealthSmart processes claims for self-funded employers. In that role it is doing the paperwork your employer’s plan requires.
As a network operator, it maintains contracted provider networks that plans use for access at negotiated rates.
Your plan may use HealthSmart for one of those jobs or both. Seeing the name in two places on your documents is expected, not a filing error.
The three networks
Our contract covers PPO, HPO and Accel. Practically, that means whichever of the three your employer selected, we are in it.
It still pays to know which one you have, because it governs every other provider you see. Specialists, imaging centers and hospitals are not all contracted across all three, and the tier your plan uses determines where you can go without a larger bill.
The network name is usually on the card. If it is not, your benefits summary will name it.
What decides your cost
Not the network. Your employer.
Self-funded plans set their own deductibles, copays and coinsurance. Two colleagues at different companies, both on HealthSmart, can have genuinely different coverage for the identical visit. When you want to know what you will owe, the answer is in your summary plan description or from the member services number, not from anything general about HealthSmart.
We can tell you whether we are in network and what eligibility returns. We cannot tell you your deductible, and any office that quotes you one confidently is guessing.

Why does HealthSmart appear twice on my paperwork?
Because it does two different jobs. HealthSmart acts as a third party administrator, processing claims for self-funded employers, and it also operates provider networks. Seeing the name in both roles is normal and not a duplicate.
PPO, HPO, Accel. Which one do I have?
It is usually printed on the card, though not always prominently. We are contracted for all three, so for our purposes the answer is the same. It still matters for other providers you see, so it is worth knowing.
Does the network I am on change what I pay here?
Your cost sharing is set by your employer's plan design, not by which HealthSmart network you access. Two people on different HealthSmart networks can owe different amounts, and the reason is the employer, not the network.
Can you verify before I book?
Yes. Call with the member ID and every name on the card and we run it on the phone.
