A small primary care practice in Southlake, built so that a routine visit is unhurried and the person across from you already knows your history.
We see patients age 16 and older.
Our mission
Know the patient, not just the chart
We exist to give adults in Southlake and the surrounding towns a primary care practice where continuity is the point. The same provider at every visit, appointments long enough to get to the real question, and follow-up that happens on a date rather than a hope.
We take insurance because primary care should not be a luxury product. There is no membership fee, no retainer and nothing to join. If your plan covers a visit here, that is the whole arrangement.
Our vision
Catch it while it is still a number
Most of what shortens lives in this county is visible years ahead of time as a number on a lab report. High blood pressure, prediabetes, rising cholesterol and untreated sleep apnea are all silent, all measurable, and all far easier to treat early than late.
Our aim is a practice where those numbers get found, explained in plain language, and acted on before they become the thing that puts someone in a hospital bed.
How we practice
Four commitments that decide most of what happens here. They are the reason visits run longer and the schedule holds fewer patients than it could.
- Continuity. You see the same nurse practitioner every visit, so nobody re-reads your history from scratch.
- Time. Forty to forty-five minutes for a first visit, and follow-ups long enough to finish the conversation.
- Plain language. Results come with an explanation, not a list of flagged values you have to interpret alone.
- Independence. Referrals go where they should, not where a hospital system directs them.

Your care team is led by Rachael Bowen, NP, with Dr. Farhan Abdullah, DO, board certified in Internal Medicine, as Medical Director.
Family Nurse Practitioner
Rachael Bowen, NP
Rachael Bowen is the Family Nurse Practitioner you will see at your appointments. She brings a decade of nursing and advanced practice experience to primary care, most of it spent with seriously ill patients, and that background shapes how she practices: thorough, unhurried, and focused on catching problems while they are still small.
Medical Director and Supervising Physician
Dr. Farhan Abdullah, DO
Dr. Farhan Abdullah is the Medical Director and supervising physician for Magnolia Primary Care. He is board certified in Internal Medicine and sets the clinical standards the practice follows.
Who you will see
Every appointment is with Rachael Bowen, NP. That is the whole point of how this practice is built: one provider who knows your history, with visits long enough to use it.
Dr. Abdullah is our Medical Director and supervising physician. He is not currently taking routine primary care appointments, and we say so plainly so nobody books expecting otherwise.
