OWNER: two logo notes.
Logos are in for Aetna, BCBS of Texas, Cigna, UnitedHealthcare, Humana, Medicare and Tricare. Texas Medicaid has no clean mark and renders as a type tile, which is normal for state programs.
These came from Wikimedia Commons, except BCBS of Texas which is the carrier's own published
asset. If you want the exact files your contracts reference, each carrier's provider portal has
them: Availity for Aetna and Humana, Blue Access for BCBS, CignaforHCP, the UHC Provider Portal,
and HNFS for Tricare. Replace the files in public/images/insurance/ and nothing else
needs to change.
Start typing your carrier or plan name. Nothing you type here leaves your browser.

No match in our list. Plan names vary a lot, so call the office at the number above with the exact name on your card and we will verify it while you are on the phone.
Reading your card
The carrier is the big logo. The plan name is usually smaller, under or beside it, and that is the part that decides whether we are in network.
Being in network with a carrier does not mean being in network with every one of its products. A carrier's PPO, HMO and Marketplace plans are separate contracts, and that gap is where surprise bills come from.
If what is on your card does not match anything in the checker, call us. Plan names vary by employer and by year, and we can verify yours in a minute.

OWNER: two carriers need their product lines specified. UnitedHealthcare and
Tricare are listed as accepted but without product detail in src/payers.mjs. For
UnitedHealthcare, say whether that is commercial, Medicare Advantage, Community Plan or all
three. For Tricare, say Prime, Select or both, and whether you are network or non-network.
Medicare
We accept Original Medicare, plus Medicare Advantage through Aetna, Blue Cross Blue Shield of Texas, Cigna and Humana. Bring your Medicare card and, if you have one, your Advantage plan card as well.
If you are on Medicare, our Medicare Annual Wellness Visit is covered at no cost to you once every 12 months.
Self-pay
OWNER: add self-pay pricing for a new patient visit, a follow-up and a physical. Patients search for this and a clear number converts well.
Before your first visit
- Call us with your plan name and member ID so we can verify coverage
- Bring your insurance card and a photo ID
- Know your copay and whether your deductible has been met
- On an HMO, name us as your primary care provider through your insurer first
Insurance questions
How do I know if you are in network with my plan?
Call the office with your plan name and the member ID from your card. Plan names are close enough to each other that guessing from the carrier logo alone gets people into trouble.
Do I need a referral to see you?
Most PPO plans do not require one. Some HMO plans require you to name us as your primary care provider first, which you do through your insurer rather than through us.
What will my visit cost?
It depends on your plan, your deductible and the type of visit. Preventive visits are usually covered in full. Sick visits and chronic care visits apply to your copay or deductible. We tell you what we know up front, though only your insurer can confirm your benefits.
What if you do not take my insurance?
Ask about self-pay pricing. We would rather quote you a clear number than have you skip care entirely.
Why was I billed for part of my annual physical?
If we spend part of a preventive visit managing an active problem, like adjusting a blood pressure medication, that portion is billed separately. It is a rule set by insurers, not by us, and we say so in the room before it happens.
See what to bring to your first visit, browse our primary care services, or read about our primary care Southlake practice.
