What this covers, and what to check
Our Humana contract covers the Medicare PPO and PFFS lines shown above. Humana also sells HMO products, branded Gold Plus in many markets, which are a separate network.
If your card says HMO, call before you book. We will verify what your specific plan shows and tell you plainly whether we are in network for it. Getting a straight answer before the appointment is worth more than a hopeful yes.
The Annual Wellness Visit, which most people miss
If you have a Humana Medicare Advantage plan, you almost certainly have an Annual Wellness Visit benefit, and there is a good chance you have never used it.
It is not a physical. It is a structured prevention visit: a health risk assessment, a review of your medications and history, cognitive screening, a personalized written schedule of the screenings you are due for, and advance care planning if you want it. There is generally no cost sharing.
People book it expecting a hands-on exam and are surprised. Others skip it because they had a physical and assume it is the same thing. Our Medicare Annual Wellness Visit page explains the difference, and we can do the wellness visit and a problem visit in the same appointment when that makes sense.
Chronic condition management
Most of our Medicare patients are here for ongoing management of blood pressure, diabetes, cholesterol, thyroid, kidney function or several of those together.
That work benefits more than anything else from continuity, which is the thing large practices struggle to provide. Keep seeing the same provider and they will know what your blood pressure was doing last spring and what we changed. Dr. Farhan Abdullah, DO, board certified in Internal Medicine, and Rachael Bowen, NP see patients here, from one shared chart.
We also offer remote patient monitoring for patients where between-visit readings actually change decisions, rather than as a device for its own sake.
Costs
Medicare Advantage plans have copays that vary by plan and visit type. Preventive services including the Annual Wellness Visit are generally covered in full. Problem visits typically carry a copay.
Call the number on your card for your specific copay. We verify eligibility and network status, but Humana confirms benefits.

Bring your Humana card, your red white and blue Medicare card if you have it, a photo ID and your medication list including supplements.

Do I need a referral with a Humana PPO?
No. Humana Medicare PPO members can see primary care in network without a referral.
What is a PFFS plan?
Private Fee-for-Service. It is a Medicare Advantage plan type where the plan sets the payment terms and providers agree to them visit by visit rather than through a standard network contract. Bring the card and we will confirm before you book.
Does Humana cover the Annual Wellness Visit?
Medicare Advantage plans including Humana generally cover the Annual Wellness Visit with no cost sharing. It is a different service from an annual physical, and it is one of the most underused benefits in Medicare.
Is Humana Gold Plus the same as what you accept?
Gold Plus is Humana's HMO branding and it is a different product from the PPO and PFFS plans listed above. Call us with the plan name on your card so we can check it against what we are actually contracted for, rather than guessing from the carrier name.
