We see patients age 16 and older.
Our office sits on Kirkwood Blvd in Southlake, just north of the SH 114 corridor and a short drive west of Southlake Town Square. If you live in Carillon, Kirkwood Hollow, Timarron, Clariden Ranch, or anywhere along the north side of town, you can be here in a few minutes without touching a highway.
Getting here
From Southlake Town Square. Head west on SH 114 or take Kirkwood Blvd directly. The building is on the north side of Kirkwood.
From south Southlake and Timarron. Take White Chapel Blvd or Carroll Ave north to SH 114, then west to Kirkwood Blvd.
From FM 1709 (Southlake Blvd). Go north on Carroll Ave or White Chapel Blvd, then follow signs toward the 114 corridor and Kirkwood Blvd.
From DFW Airport. SH 114 west, roughly fifteen minutes depending on traffic.
Parking and the walk in
Free surface parking directly in front. It’s a short, flat walk to the entrance with no stairs and no parking garage to circle. If you have mobility trouble, park in the spaces closest to the door and someone can help you in.
Why Southlake patients come here
A lot of people in this area technically have a doctor and haven’t seen one in three years, usually because getting an appointment means calling a large system and waiting a month. We’re a small practice, which means same-day sick visits are realistic and you see the same nurse practitioner each time.
Rachael Bowen, NP handles the appointments, and Dr. Farhan Abdullah, DO is our Medical Director. You see the same provider every visit, which is what makes continuity possible.
What we do and don’t do
We see patients age 16 and older. We handle annual physicals, chronic conditions like diabetes and blood pressure, same-day sick visits, and Medicare Annual Wellness Visits.
We don’t provide pediatric care, prenatal care, or emergency care. For an emergency, call 911.

We share the Magnolia name and Dr. Farhan Abdullah, DO serves as Medical Director for both. Magnolia Primary Care is a separate primary care practice with its own suite, its own phone number, and its own scheduling.
Do you take care of Carroll ISD students?
We see patients age 16 and older, so we can handle high school sports and school physicals. We don't provide pediatric care for anyone younger.
